  • Valid Matric or Equivalent Certificate with ‘O’ Level Transcript.
  • Valid F.Sc. or Equivalent Certificate with ‘A’ Level Transcript.
  • Domicile Certificate.

  • Four Passport size photographs with sky blue background.
  • PMC entry test marks certificate
